Patients are required to bring all insurance information with them the day of their appointment. You will need to bring your insurance card for us to photocopy. All insurance co-payments are due at the time service is rendered. If you need a referral form from your general dentist you must have that with you. If you do not have the referral form we will not be able to see you.
