For your convenience, we have referral forms. It is helpful to the doctors if you use these forms. Please contact our office if you need some of these forms.
If it is not an emergency, it is easier for the patient to contact the office to set up an appointment. They will have all the information that we need on the referral form.
If the patient has dental insurance that requires a referral form, it must be filled out completely by your office. This includes charge schedule and doctor office code.
In most emergency situations your patients will be seen on the same day. It is most helpful to us that when you call you provide us with the following information:
Patient's name
Telephone number
Does the patient pre-medicate
Is the patient on blood thinners
Are you sending an x-ray
Tooth number/area
Any other information that we may need to treat the patient
The doctors would appreciate that if a tooth was started or there are any "special" situations (perforation, separated file, etc.), or if the patient has any medical problems that need special attention, that you contact them to discuss the case. This way the patient can be scheduled accordingly.
